An exciting opportunity has arisen to join the Whittington Health NHS Trust Children’s Complex Care Team. The Continuing Care Team co-ordinates and provides direct care into the homes of children and young people (CYP) who have complex life threatening/ life limiting conditions across the boroughs of Camden, Islington and Haringey. These CYP have a range of conditions and needs which can affect their ability to breath, move independently and communicate. We also support a growing number of children with behaviours that challenge. These CYP require our care and support to be able to live their lives to the fullest and enjoy experiences. The post holder will be responsible for supporting the facilitation and organisation of care, as well as some direct care, for CYP with continuing care health needs. They will be responsible for effective assessing, planning, implementation, delivery, and evaluation of care to CYP, under the direction of the Senior Practitioner. The post holder will support training and development of carers to ensure all aspects of care are delivered effectively and safely to an allocated caseload. The post holder will work closely with the community nursing teams to ensure effective communication and care. They will develop skills and knowledge in the use of the National Continuing Care Framework to carryout assessments and reviews. The post holder will support the Senior Care Team to continue the strategic development of the service.
You will be expected to supervise, teach and support a team of nurses and carers that provide health care support for children receiving a continuing care package. You will need to work clinically with carers within various settings including the family home. Shifts will vary according to the package and training requirement and will include night duty.
You will need to ensure that nursing practice is consistently of highest quality and based on up to date research in all community settings to include the child’s home. You will need to clinically assess new referrals using the DH Children’s Continuing Care framework. You will play a pivotal role with the process of transfer and discharge of the child from hospital to home.
You will work in partnership with the nursing team to ensure records of care training and required competency booklets are up to date to enable carers to deliver safe and effective care.
You will support the young person and their family in transition to adulthood. To help in the organisation of MDT’s and attend meetings representing the team.
Medical services have been delivered on the Whittington Hospital site for over 500 years. On 1st April 2011, Whittington Health was launched as a new Integrated Care Organisation (ICO) comprising The Whittington Hospital NHS Trust, NHS Islington and NHS Haringey. The Whittington and the community health services in Islington and Haringey are award winning organisations, delivering acute and community based health services to a population of 443,000 people.
The organisation is the biggest employer in the area, with over 4,500 staff; delivering healthcare not only on the acute hospital site which has 420 beds, but also from a variety of other community based health centres and through visits to patients homes across the boroughs to address the health needs of the local population. Whittington Health also works with other health, social care and voluntary sector partners to support patients and service users from their initial appointment whether it be with a community health team or at the hospital, all the way through to treatment and tailored after care.

The post holder is expected to work within the general Children's community Nursing team. Responsible for providing skilled nursing care for sick children in their own homes or other suitable venues that would otherwise be performed in an acute hospital setting. They are also responsible for the effective assessing, planning implementation, delivery and evaluation of individualised nursing care to children as required and under the direction of the Senior Practitioner and Hospital at home lead.
The post holder will support the senior staff and the Matron to continue the strategic development of the service. They will help to foster and build effective working relationships within the service; children’s nurse specialists, the community children’s nurses, and across multi-agencies including E.D. staff and general practices.
