Job description
Undertake case finding initiatives to identify patients with hepatitis C, working with Hepatology nursing team, Hepatitis C trust peers, Hepatology Data team, GP practices, laboratory databases and other organisations Liaise with patients to encourage them to enter care and to manage their pathway, ensuring a convenient date, time and location is agreed To appropriately remind clients of their appointment to facilitate care. This may include sending reminder letters or contacting patients by telephone, or text Facilitate the prioritisation of HCV positive individuals, by supporting the coordination of assessments at outreach clinics and linkage into care pathways Lead the development of protocols and materials to support the training of outreach/community partners Manage database of HCV positive individuals in order to track treatment journeys Support the Viral Hepatitis team by taking blood for HCV testing and Fibro scanning patients at outpatient and outreach clinics within Sussex To motivate and encourage staff within outreach clinics e.g. drug and alcohol services to routinely offer HCV testing by capillary, oral or dry blood spot testing Monitor the performance of HCV testing by organisations in the community e.g. drug and alcohol and sexual health clinics Maintain a record of HCV training delivered within the ODN; including competencies, monitoring and evaluating effectiveness of training
