Please see the attached Job description and person specification for more details. Professional: 1.1. To adopt and co-ordinate a multi-disciplinary agency approach to care, ensuring that all aspects of the patients needs are met. 1.2.
To set professional standards of care and to monitor, measure and audit the effectiveness of care given. 1.3. To ensure an effective communication mechanism is in place, particularly with regard to patients care and progress, to include patients and relatives. 1.4.
To deploy, supervise and support other members of the nursing team in the provision of a professional nursing service. 1.5. The utilisation of available resources and services within the community setting, identifying any inadequacies and taking appropriate action to enhance the quality of care available within their client group.
