Job description
This is a fantastic opportunity to help lead and influence continuous improvement in palliative and end of life care across the communities we serve. We are looking to recruit 4 innovative and motivated senior nurses across the 4 localities of Birmingham ( North, East South & West) who are passionate about delivering high quality palliative and end of life care for our patients and their families.
With a focus on clinical leadership and quality improvement, the post holder will:
- Support community teams to deliver evidence based, pro-active individualised care; acting as a role model and enabling staff to develop confidence and competence in all aspects of generalist palliative and end of life care
- Identify training needs within community teams and facilitate local learning
- Lead on the implementation and ongoing review of local quality improvement initiatives
- Work in collaboration with our system partners e.g. Hospices and acute Trusts to support development, implementation and ongoing review of system wide discharge pathways relating to palliative and end of life care

Independent Non-Medical Prescribing is an essential requirement of this role and applicants must have completed this course (V300)
The role will involve direct clinical care but the post holder will not hold a clinical case load. Applicants must have experience of delivering palliative and end of life care in community settings
