Job description

Come and join our CQC rated ‘Outstanding’ Trust as a Community Nursing Sister/Charge Nurse for the Windsor and Maidenhead Community Nursing Team. As a Community Nursing Sister/Charge Nurse you will manage a clinical caseload under the delegation of the team leader as well as exercising leadership and management of a defined community DN team to ensure good clinical practices and high standards of care are maintained in order to meet service demand. Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence-based practice. You will have a passion and enthusiasm to care for patients in their own homes.

  • Maintain required competencies, as identified in the Band 6 Community Nurse competency framework, in order to effectively undertake patient assessment, care planning and immediate care needs.
  • Support staff to complete holistic assessment and reassessment of all health and nursing needs, ensuring that every patient has an overview of health and nursing need recorded.
  • Manage and be responsible for a clinical caseload under the delegation of the team leader.
The “must haves” for this role:
  • Hold a full UK Driving Licence and have use of a car for work purposes.
  • A passion for community nursing, that matches our passion to provide excellent healthcare for patients within their own homes.
  • Willing to undertake training to fulfil the requirements of these role.
  • NMC current pin.
