Job description
Are you a Registered Nurse with excellent team working skills who would like to join the District Nursing Team working as an Out of Hours Community Nurse.
The successful applicant will provide professional, holistic nursing care and support to patients , carers and families within the community setting, delivered with compassion, dignity and respect.
This post will require an individual who delivers the highest quality care, ensuring evidence based/ best practice and has a commitment to continuous professional development.
Applications are invited from registrants with excellent communication skills, who have a flexible and adaptable approach to changing work patterns. We have 1x 30 hour post and 1x20 hour post available.
We offer mentorship, working life balance and professional development opportunities to meet your need.
They will be involved in direct delivery of care and overseeing nursing care to patients in their own homes and other community settings. They will deliver care to complex patients in the community.
The staff nurse will be required to provide professional nursing care and support to patients, carers and families in the own home setting.
The member of staff will be required to function as a member of the multi-disciplinary team, assess individual patient’s care needs and subsequently develop, implement and evaluate appropriate plans of care.
The members of staff would be required to attend training to maintain mandatory skills alongside developing new skills to meet the provision of care required.
