Job Summary
Care navigator role: To act as a care navigator, to provide a point of contact for patients and to act as a focal point of communication between patients, doctors and other medical and clerical staff.
Administrative role: To ensure medical records are kept accurately and to receive, action and workflow incoming letters and clinical documents.
Working Hours: 34 hours
- Monday 8.00am – 3.00pm (includes 30 minutes unpaid break)
- Tuesday 12.00pm - 6.30pm*
- Wednesday 12.00pm - 6.30pm*
- Thursday 8.00am – 1.00pm
- Friday 8.00am – 6.30pm (includes 30 minutes unpaid break)
*Negotiable number of hours, but must finish at 6.30pm
Main Duties and Responsibilities
· Greet patients coming into the surgery and direct them appropriately
· Create and update the appointment book under the supervision of the Reception Manager
· Ensure that patients requiring appointments are seen in a logical manner according to urgency.
· Run the surgery appointment system effectively
· Accurately record baby immunisation clinics onto the appointments screen & liaise with child health in relation to changes to clinics
· Run clinics effectively
· Answer calls to the surgery number and find the patient the appropriate service
· Follow up patients who do not attend for their appointments at the surgery
· Follow up of patients who do not attend hospital appointments
· Follow up patients who do not attend for retinal screening
· Take appropriate details from patients wishing to register with the practice and provide appropriate information about the practice
· Inform patients of their named GP
· Ensure up dated ethnicity codes are on all patients records
· Explain practice arrangements to patients and those seeking temporary cover.
· Accept and register patients in accordance with practice procedures
· Update patient demographic information when changes are identified.
· Advise patients of relevant charges for private services, accept payments and issue receipts.
· Respond to queries and request for information from patients and other agencies.
· Adhere to information governance policies and procedures
· Entering patient information onto the computer as required, in particular in relation to the Read coding of medical conditions
· Sorting and summarising of medical records
· Using docman and docman workflow for attaching, scanning of correspondence, reports, results ensuring scanned in correct record and any action to be taken is dealt with, particularly with regards to results to be seen by the duty doctor the same day
· Photocopy as requested
· Take requests for prescriptions accurately and enforce the repeat prescribing policy.
· Produce acute / repeat prescriptions under the supervision of the Practice Pharmacist
· Promote on-line access for booking of appointments and requesting of repeat prescriptions
· Record and book requests for ambulances.
· Ensure the reception and waiting areas are kept neat and tidy
· Ensure infection control procedures are maintained with regard to cleaning of the reception, admin and waiting areas (including the cleaning of vomit/faeces in accordance with practice procedures)
· Completion of hazardous waste forms on a weekly basis
· File and retrieve medical records as required, ensuring strict alphabetical order.
· Ensure that correspondence (paper and electronic) are directed quickly and correctly.
· Receive and make telephone calls as required / to accurately record messages.
· Ensure night service is taken off the telephones each morning and placed in operation at the end of each day.
· Open the surgery premises and prepare for surgery
· Secure premises according to practice guidelines.
· Document all requests for reports to be completed by the GP
· Complete referrals to the district nurse team under the supervision of the GPs
· Process the actions recommended by clinicians from hospital letters and pathology/radiology
· Produce pathology requests as directed by the clinician
· Maintain awareness of target patient groups e.g. GSF, ILCT, Admissions Avoidance
· Support QOF/QIF and enhanced service targets as directed by the practice manager/Reception manager
· Follow instructions given by the Practice Manager and Reception Manager.
· Give out patient questionnaires to patients for surveys being undertaken
· Undertake specific data entry or admin. projects as instructed by Practice Manager/reception manager
· Act as a first point of contact for key areas and direct patients appropriately
· Frank post appropriately, and take post when required
· Adhere to all practice policies procedures and guidelines.
· Comply with the standards set out in the contract of employment.
· Undertake any other duties as appropriate to the role.
