Nexxus Care are looking for a passionate and compassionate person to join our Reablement Service as a Support Worker.
As a Support Worker, you will be responsible for supporting individuals in their own home. The role of our Reablement Service is to provide support to individuals in their own home, our aim is to support individuals to prevent an admission to hospital and our service is also put into place for a 6 week period on discharge from hospital, to enable individuals to gain back as much independence possible.

Job Description:
As a Reablement Support Worker you are responsible for providing support to customers within their own home and surrounding community. The aim is to provide support and promote independence to enable individuals to be as independent as possible so they can live safely within their own home. This position requires a high level of attention to detail, excellent communication skills, good knowledge of the care sector, and the ability to work independently or in a team environment.
Whilst this job is based in Cannock, you will be expected to travel within the Staffordshire area - some areas include Tamworth, Lichfield, Rugeley and South Staffordshire. With our Reablement Service you are paid block pay, therefore you are paid from the time you start your shift to the time you finish. You are also paid mileage at 35p per mile, you are also paid mileage to your first call from your home address and from your last call to your home address MINUS the first 5 miles.
Essential Duties and Responsibilities:
- Provide care and support to individuals in their own home
- Support and Promote Independence
- Maintain care plans and appropriate documentation
- Answer phones in a professional manner.
- Undertake personal care
- Assist with continence care e.g. changing incontinence pads, catheter leg bags and night bags
- Administer medication as per the prescription
- Perform other duties as assigned

- Strong work ethic.

- Ability to work independently or in a team environment.
- Ability to work using your own initiative but not working outside of your competence level
- Ability and willingness to travel across a large area
