Job Title: Health Visitor/Specialist Community Public Health Nurse

Pay Rates: Day £20.76, Nights/Saturdays £26.76, Sunday/Bank Holidays £33.21 (per hour)

What you’ll be responsible for:·
To lead delivery of the Healthy Child Programme including the 6 High Impact Areas within the community using evidence-based practice within the four service offers.
· To provide effective leadership and management to staff, that promotes high performance standards both individually and as a team, in the achievement of the Trust’s objectives.
· To identify and assess any children and families in need, and to work in partnership with Primary Care, Children’s Centres, Children’s Services, Voluntary Sector and other appropriate agencies, to deliver integrated programmes of care within the local area.
· Work within Hampshire Safeguarding Children’s Board and Southern Health NHS Foundation Trust (SHFT) policy and procedures, prioritising attendance at Case Conferences, reviews and planning meetings. To be responsible for the provision of case conference reports and court reports as required, contributing to safeguarding of all children.
· To provide a continuum of support according to need, improving outcomes for children by reducing health inequalities. To support the Early Help agenda to ensure early intervention.
· To be aware of the Key Performance Indicators for the Children’s Division and focus service delivery towards their achievement.
· To be responsible for the quality and management of the RiO electronic patient record and data reporting system within the team.
· To be the principal point of contact for allocated GP practices and Children’s Centres within the area. To develop and deliver integrated working arrangements with these services that support healthy outcomes for children.
· To promote and enhance health and wellbeing to children and their families through assessment, implementation of individualised health care plans and evaluation of care.
· To identify and assess children 0-4 with complex health needs, offering an enhanced integrated service within the multi-agency arena, enhancing equality and opportunity for them.
The role will involve, but not be limited to you performing the above.
You’ll have the following qualification/experience:
Registered Nurse/Midwife
BSc Hons Public Health Specialist Practitioner (Health Visiting) or equivalent
NMC Registered Part 3
Nurse Prescriber
Full UK Driving Licence (and access to a car)
Child Branch Registration or similar qualification
Mentorship Module
Evidence of ongoing learning and professional development
Experience:
Working in the NHS and experience of leading/managing skill mix teams
Experience of working collaboratively with other agencies.
Experience of working with individuals in a health promotion role
Experience of managing own time and diary
Experience of mentoring students
Child Branch Registration or experience of
working with children
Previous experience of working in a community setting
