Job description
The post holder will play a key role in supporting the All Age Continuing Health Care & Individual Patient Activity service across the region.
The post holder will be involved in the project management support to deliver an effective and competent level of service, consistently delivering a client-focused service.
This is a development post within the All Age Continuing Health Care Service with competencies set for progression to Band 6
Key Responsibilities
- Support the Associate Director of Continuing Healthcare and Project Team members to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively.
- Provide high quality project, service, initiative and administrative support including information and analysis.
- The post holder will undertake reporting and analysis of information to support delivery.
- The post holder is a key member of the team whose overarching goal is to enable, promote and support the effective use of data, information, knowledge and technology to improve, inform and support a portfolio of projects, services, and initiatives.
The post holder will:
- be required to maintain constructive relationships with a broad range of internal and external stakeholders.
- participate in relevant internal and external working groups/projects, services and initiatives to provide, information and analytical advice to strategic leads.
- work with members of the Team to develop and implement project data collection systems that will provide accurate and timely data.
- communicate information and issues, including briefings and reports, to manager, strategic lead/ strategic manager as appropriate.

Functional Responsibilities:
Project Management
- Undertake information/project analysis as agreed with manager.
- Contribute to effective information management within the team.
- Participate in relevant internal and external working groups/projects to provide information/analyst advice and support and to maintain data collection systems for its effective use by the team.
- Analyse and report on data and monitor the processing of data and information.
- Provide information to the project lead on project and statistical information matters.
- To progress actions with the project team members and produce progress reports.
- To provide project management support and expertise.
- Support and inform the requirement and targeting of resources, monitoring, implementing and evaluating projects and delivery of financial benefits and providing high quality information and analysis.
- Contribute to the financial delivery of the agreed portfolio ensuring it is delivered on time.
- Provide training, advice and support on own area of responsibility
- Line management of Admin Support Officers.
- Carry out timely and accurate information analysis and reporting on agreed areas of portfolio and present findings in an agreed manner.
- Develop and maintain databases and project management systems as required by job.
- Maintain administrative and information resources.
- Undertake auditing of projects, services and initiatives.
- Carry out web based and publications research.
- Actively support and contribute to the development of key performance indicators for the successful assessment of performance.
Planning and Organisation:
- Support implementation of project, service, initiative through timely and relevant information analysis and administrative support, in accordance with the agreed priorities of the Team.
- Deliver against agreed objectives, achieving quality outcomes.
- Organise meetings or events and assist in the diary management requirements of individuals in connection with portfolio of work.
- Support with the facilitation of meetings.
- Support with secretariat duties including minute taking and project board support.
- Work with the project leads to ensure project plans, risks and issues registers and action trackers are kept up to date and key actions are followed up on.
- Propose changes to own project, service, initiative work, informing policy and making recommendations for more effective delivery.
- Contribute to the review and development of existing project information management systems and contribute to the development of an integrated approach to project management.
- Keyboard skills
- Access to vehicle for occasional travel to visit individuals, providers and other sites
- Providing accurate and legible recording of information
- Ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures
- The post holder will be required to undertake any other duties and responsibilities within the framework of the post.
- The post holder will be expected to keep up to date with local, regional, national and international developments both within the NHS, Social Care and externally.
