Purpose of the job
The overall purpose of this post is to lead the delivery of ‘A Good Life’ service and associated projects provided by KDC in partnership with local partner agencies. The Project Coordinator will ensure the successful delivery of wrap-around support for adults in Knowsley to resolve issues, build resilience and move forward towards independence and ‘a good life’.
The role involves receiving, prioritising and allocating referrals, maintaining records of outcomes and working with partners, who use person-centered approaches to help individuals to deal with the immediate issue(s) they are facing and work together with them to focus on addressing underlying causes and long-term problems.
The post-holder will take responsibility for the continued development and promotional activities of the project and implement and development plans.
Key Duties andresponsibilities
- Receive referrals, triage and assess individual cases ensuring they meet the criteria of the project
- Prioritise referrals and allocate to the partner agencies
- Maintain database of client information utilising a CRM system
- Ensure services provided are EDI compliant
- Produce quantitative and qualitative reports and case studies evidencing outcomes and impact and performance and quality standards
- Produce Quarterly monitoring information and reports to meet the National Lottery Community Fund requirements
- Attend Project Board meetings and deliver progress reports and updates
- Develop working relationships with staff teams from partner organisations
- Establish strong links and build networks of relevant organisations across the Borough of Knowsley, including statutory services, voluntary projects and community groups
- Maintain contact with clients and deal with all incoming calls and queries about A Good Life Project
- Maintain awareness-raising and promotional activities to improve the profile of A Good Life Partnership and increase referrals – including the use of social media
- Use the Outcomes Star™ assessment model to measure and capture progress
- Record and report outcomes to ensure agreed
- Liaise and work in partnership with colleagues, professionals and groups to deliver person-centered outcomes
- Gather and disseminate information within the community to bring people and opportunities together
- Become a recognised presence and point of contact within the community
Annual leave: 30 days rising to 33 maximum plus statutory bank holidays
