An opportunity has arisen to work with our continual development of the Programme Coordinator role in supporting process and procedures in the integrated work of Home First Team in supporting avoiding hospital and care home admission and facilitating hospital discharge.
The role is currently working Monday to Friday rostered between the hours of 7.30am and 7pm and on a further rota basis at weekends & bank holidays. You will be based at our Assessment Hub Perth House but may be needed to work at our other locations across the City.
You will be working closely with colleagues as part of our vibrant multi- disciplinary team, supported by our Pathway 1 Community Registered Manager. You will be responsible for receiving new referrals into our service areas, collating and providing performance data with reporting and data input on our local government systems, with the ability to analyse situations. Key to this role is the geographical planning of staff rotas and customer visit allocation across Derby City using our electronic rostering system. You must be able to work creatively to specific timelines in our fast paced, pressured environment to ensure our service runs smoothly with adequate staffing and customer visits met.
