WHAT IS A JOB TITLE
As part of the Street Outreach Team, the Program Manager performs implementation and compliance with contracts and PATH’s
integrated service approach. The Program Manager will participate in daily street outreach to homeless clients in the San Jose community. Position Responsibilities:
Supportive Services
- Provide daily street outreach services to build rapport and bring services to clients in the community
with the goal of linking/connecting them to long term supportive services.
- Assist the client with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth
certificate, social security cards, provide employment linkages and assistance, housing referrals and health
related assistance as identified on the ISP.
- Meet with established clients on a weekly basis to review, evaluate, and support completion of their goals.
- Respond within 1-3 business days to requests for street outreach services and send follow up information to
requestor and supervisor.
- Conduct a VI-SPDAT (Vulnerability Index- Service Prioritization Decision Assistance Tool) on all new clients to
connect them to the proper intervention and services. Ensure all data is entered in a timely manner.
Program Management and Documentation
- Participate in and lead staff meetings and trainings as assigned by your supervisor.
- Provide administrative support including data analysis and reporting.
- Support leadership through contract growth and project developments including the planning an
coordination of projects and its activities and the on boarding and training of new staff, interns, and
volunteers.
- Coordinate team members to ensure community requests are addressed in a timely manner
- Provide weekly supervision sessions to staff members, offering support and insight to client engagement
strategies.
- Help build and maintain positive relations within the team and with external parties, foster positive team
culture and practice professional leadership.
- Produce and evaluate reports, decisions, and results of the Street Outreach Team and recommend new
approaches, policies, and procedures to effect continual improvements in operations and efficiency.
Documentation
- Complete paperwork on every face to face/telephone contact with client or collateral contact by the next
business day at 5pm.
WHAT YOU BRING
We’re looking for candidates that:
- Able to work independently and as part the team and exercises mature and professional judgment.
- Strong written and verbal communication skills.
- Good problem solving and conflict resolution skills.
- A highly motivated self-starter and ability to coordinate multiple projects/tasks at once.
- Knowledge of maintaining and executing confidential information using HIPAA standards.
- Computer skills with proficiency in Microsoft Office software.
- Flexible, adaptable and have the capability to work in a fast paced, professional environment.
- Able to work alternative work schedules, evening and weekends as needed for operations of Street Outreach
and at the request of management
PREFERRED QUALIFICATIONS
- Master's Degree in a related field; Strong preference for MSW or MFT
- At least two (3) years of experience with working in a homeless street outreach program.
- Lived experience with issues related to homelessness, mental health, or substance use recovery.
- HMIS experience
- Experience with program start up
- Experience analyzing program policies and procedures and practices to identify barriers and impediments for
achieving goals and making recommendations for course correction.
MINIMUM QUALIFICATIONS
- Bachelors in related field with at least three (3) years of experience working with highly vulnerable
population, particularly those with homelessness, health, mental health, and substance abuse conditions.

At least (seven) 7 years of experience working with highly vulnerable populations, particularly those with
homelessness, health, mental health, and substance abuse conditions.
- Demonstrated knowledge and or experience with advanced and evidenced based interventions, including
engagement and rapport building with hard-to-reach clients, Harm Reduction and Motivational Interviewing
strategies, Housing First, non-violent crisis intervention techniques and case management interventions
effective with a vulnerable population.
- Previous experience in social services, homelessness programs, and/or working with vulnerable population
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
