Wellbeing Assessor (Primary Care) (Winchester)
Service: Wellbeing (E&W)
Contract: Permanent
Hours: 30 hours per week (may include Saturday mornings/evenings/bank holidays)
Salary: £25,071 to £27,677 (pro rata) per annum
Location: GP Surgeries (Twyford, Bishops Waltham, Stokewood, Wickham)

About the role
This service has been developed to enhance the mental health support which GP’s are able to offer their patients through a collaborative and integrative approach across GP practices and associated services.

About you
In this role you will undertake assessments of patients in order to understand their individual needs and identify the most appropriate support for them, working collaboratively with other partners in the primary care mental health service and community-based organisations.
