Job description
Assess, diagnose, plan, implement and conduct treatment and or interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs within the domiciliary setting.
Working out of a busy GP hub in Sheffield you will be conducting home visits to patients as requested. Visiting these patients, assessing and creating care plans alongside on-call GP Doctors to give patients the best care and remain home where possible.
You will see a wide variety of pre-screened calls throughout the day and are expected to be able to conduct a professional and swift assessment, create a treatment plan and then document notes onto System One.
Hours:
Monday - Friday: 09:30-17:30 (8 hours fully paid) or 11:00-16:00 (5 Hour shifts, times are semi-flexible for this shift)
Saturday & Sunday: 11:00-15:00 (4 Hour shifts, Paid flat rate £43.75ph + any OT)
This is a casual contract so workers may choose as many or as few shifts as they wish, Pay rates vary depending on Self Employed or PAYE staff and mileage is reimbursed.
