York CVS is looking for a special person for this new role within our social prescribing team. We are looking for someone with an inquisitive mind, who can independently identify which patients would benefit from our support. You will have a natural ability to build professional relationships with GP’s, nurses, patients and colleagues both internally and across the voluntary sector to create positive outcomes for our patients in York by addressing health inequalities.
As an advocate for patients and an ambassador for York CVS, you will not be afraid to challenge in the best interests of the people you are supporting.
