Job description
Our Crisis Service covers Havant, Waterlooville, Bordon, Petersfield, Fareham and Gosport, therefore a car driver is essential. This role will be focused on working within our Crisis and Assessment Team, providing a 4 hour response to service users who present in a self defined crisis, to complete holistic assessments and make a comprehensive decision on treatment required such as Intensive Home Treatment Team or admission to Hospital.
As a member of the Crisis Urgent Assessment and Home Treatment Team, you will ensure that you offer responsive, evidence-based and recovery focused care to the service users. You will work as part of a multi-disciplinary team, which supports individuals at home who are experiencing an acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible, whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home.
