Job description

Our busy General Practice is looking to appoint an experienced Practice Nurse to join our friendly team working alongside our Practice Nurses, HCA and Phlebotomist.

We seek candidates experienced in treatment room duties, cervical cytology, wound care, vaccination and immunisations, health promotion and areas of chronic disease management. Training offered.

We are looking for a well organised and proactive nurse to compliment the existing members of the team. Knowledge of the SystmOne clinical system is desirable.

We seek candidates to work part time and the hours can be flexible.

Cover for colleagues during absence is required.

Job Purpose

To work as part of the Practice Nursing team to provide nursing care for patients including treatment, preventative care, screening and patient education.

Specific Responsibilities

  • Ensure all patients are treated with dignity and respect by you, ensuring appropriate face-to-face and telephone manner is used with patients and carers thus promoting a pleasant and positive image of the Practice
  • Assess, diagnose and treat specific diseases in accordance with agreed medical nursing protocols.
  • Evaluate clinical results and implement individual patient pathways in response to findings based on clinical guidelines.
  • Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy.
  • Discuss the implications of diagnosis with patients and carers providing advice, support and information about lifestyle changes.
  • Manage programmes of care for patients with acute and chronic diseases.
  • Assist in the monitoring and improvement of drugs compliance, supporting patients and carers.
  • Perform routine treatment room duties such as wound care, cervical smears, immunisations, injections (such as B12s), ear irrigation, processing of samples, ECGs, phlebotomy, suture removal, etc.
  • Manage stock levels and ensure treatment room equipment is in good working order and maintained annually or more frequently as required.
  • Perform a range of clinical skills within clinical competence.
  • Establish and maintain effective recall and monitoring systems (register) for patients with chronic diseases.
  • Communicate with patients, carers and relatives issues regarding confidentiality, consent, choice and patient rights.
  • Supervise HCA and Phlebotomist when necessary in absence of Lead Nurse.
  • Act as clinical expert in areas of special interest, offering advice and education to members of the clinical team.
  • Ensure the clinical team is updated on developments within the chosen areas of special interest.
  • Provide lead for infection control issues and training for Practice staff.
  • Ensure all appropriate care pathways, patient group directives, protocols and procedures are in place and review annually.
  • Ensure personal knowledge and expertise is up-to-date in area of special interest, attending courses/training days as necessary.
  • Ensure record keeping is timely and accurate, particularly relating to patient consultations.
  • Contribute to/lead clinical audit activities as agreed by clinical team.
  • Participate in an annual appraisal and identify individual goals.
  • Maintain Personal Development Plan.
  • Keep up-to-date with developments in countywide specialist services.
  • Understand legal issues concerning child protection including being aware of statutory child health procedures and statutory local guidance.
  • Undertake research based projects as required.
  • Provide chaperone services as required
  • Organising patients for clinic recalls
  • Work with the reception team to provide timely Health Promotion displays
  • Complete other tasks relating to QOF and other enhanced services the Practice is committed to, completing all relevant paperwork, recording and audit.
  • Attending meetings and training events as required.
  • Contribute to and develop protocols and procedures as required.

  • Alert other team members to the issues of quality and risk in the care of patients
  • Ensure own actions are within levels of competence and consistent with clinical governance systems and bring to the attention to the GP Partners any specific risk situations
  • Practice in accordance with agreed standards of care
  • Enable patients to access appropriate professionals within the team
  • Ensure stock items under your control are ordered and available in the treatment and consulting rooms
  • Be fully conversant with Practice Policies and Procedures.
  • Participate in Practice meetings, giving feedback or contributing to systems or plans aimed to improve patient experience
  • Be aware of and meet the requirements of the GMS Contract and QOF related tasks.
  • Participate in any clinical audits

Customer Service

  • Provide advice and support to patients who contact the Surgery, updating Clinical Team and Practice Manager of any important issues whenever necessary.
  • Liaise with other organisations and clinicians in a professional manner at all times ensuring the image of the Practice is not compromised.

Logistics

  • Work constructively with colleagues to ensure adequate dispensing staffing levels, co-ordinating annual leave and other requests to maintain an effective service.

Information

  • Handle patient and practice sensitive information in strict confidence, ensuring any information for shredding is disposed of correctly.
  • Ensure data handling collation and storage is undertaken in line with statutory and Practice guidance, Data Protection Act, Caldicott Guardian, Freedom of Information and GDPR requirements.
  • Record all information and activities undertaken with patients and carers in an accurate and timely fashion using manual or computer systems as appropriate
  • Follow procedures as set out in the Practice Confidentiality Policy

Facilities Maintenance and Management

  • No current responsibilities for alarm or locking building.

Personal and People Development

  • Take responsibility for own developmental learning and performance including attending training (formal and in house)
  • Maintain record of own personal development
  • Work with the Practice Manager and other clinical team members to ensure you receive appropriate training and are skilled to carry out required tasks
  • Be aware of legal issues/areas of competence associated with the Post
  • Participate in annual appraisal process of own performance to agree personal training and development needs

Health and Safety

  • Ensure own personal safety when dealing with actual or potentially abusive situations.
  • Identify Health and Safety risks and maintenance issues for environment, reporting to Practice Manager where appropriate
  • Deal with extremely rare exposure to bodily fluids in a safe manner using appropriate personal and protective equipment
  • Follow appropriate infection control procedures and maintain work areas in each clinical room
  • Ensure safe storage of cold chain items including vaccines within area of responsibility
  • Undertake basic life support training regularly and be aware of basic first aid
  • Be aware of local child protection procedure
