College Health Ltd is a leading Primary Care provider operating a network of GP Surgeries across Grays and Tilbury Essex. We are innovative in our approach and are known for providing high quality Primary Care services to in excess of 60,000 patients.
We are seeking a dynamic, resilient, Practice Nurse (and/or Practice Nurse requiring further development) to join our growing multidisciplinary team, consisting of GPs, ANP, Clinical Pharmacist, Specialist Chronic Disease Nurses, Paramedic, Mental Health Practitioner and HCA, together with a strong and experienced management and admin team able to support the delivery of excellent care to our patients.
The post holder will be flexible, hardworking and empathetic and responsible for the delivery of practice nursing activities to the practice population. They will work collaboratively with the Nurse Manager and general practice team to meet the needs of patients, supporting the delivery of quality QoF and enhanced services, maintaining safe infection control processes, supporting the delivery of immunisation (both child and adult) and cervical screening programmes, safeguarding, policy and procedures, and providing nurse leadership duties as required.
You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the Lead Nurse and Partners to deliver organisational priorities in our Practice locations in Essex.
Clinically, the focus of the role is management of patients with long-term conditions and general nursing interventions for all patients. You will be supported by a strong MDT & clinical team working.
Perform any investigatory procedures requested by clinicians.
Participate in the administrative and professional responsibilities of the practice team, ensure accurate notes of all consultations and treatments are recorded in the patient notes.
Applicants must possess a RGN and NMC qualification.

You will also be required to provide a full occupational immunisation record upon and an enhanced DBS check will be carried out.
