Job description
This is a full time community post; the post-holder will provide input into the Rotherham Older People’s Memory Assessment Service and it is expected that clinical contact (DCC) will comprise 80% of the working week with the other 20% dedicated to administration, CPD, service development and teaching dependent on the special interests of the successful candidate (SPA).
Rotherham Memory Service provides multi-disciplinary assessment for the diagnosis of dementia and delivers post diagnostic counselling, advice and treatment along with nurse prescribing services for patients over the age of 65 and their carers. The service sits within our wider Older People’s Mental Health (OPMH) community service model.
The OPMH community teams have a multi-disciplinary composition which comprises, Consultant Psychiatrists, Clinical Psychologists and Cognitive Behaviour Therapists, Nurses, Occupational Therapists, Vocational Workers, Social Workers, Support Workers, Administration staff and Managers.
To work with other members of the Memory Assessment Service to provide a comprehensive service to the patients under the care of the team. This will involve formulation, care planning, medication management, diagnostic review and medical support to other clinicians.
To work in collaboration with the team manager to ensure an excellent level of care for patients there, this will involve weekly MDT for the purpose of reviewing referrals, medication management, oversight of holistic treatment plan and future care/discharge planning
