Physical Therapist Assistant: Home Health
Multiple Full Time Positions Available

We provide care to over 1,600 patients in San Francisco, San Mateo, Santa Clara, Alameda and Contra Costa Counties. Currently expanding to Solano/Napa Counties.

Territory: Hayward, Castro Valley, San Lorenzo and San Leandro as well as Fremont, Union city and Newark. Will be expanding to the Tri-Valley including Pleasanton, Dublin and Livermore.
What you’ll do as a 21st Century Home Health Physical Therapist:
Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment. You will:
- Examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.
- Work with patients to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

To be a success as a 21st Century Home Health Physical Therapist Assistant
- License PTA in California
- Home Health experience is highly preferred
- Current CPR License
- Current CA Driver's License
- Computer Proficiency
- Strong organization and communication skills
- Work well independently and in a team
- Excel at building rapport with patients and providing the highest quality of care
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Self-Directed

Pay: $85,000.00 - $135,000.00 per year
