Job Summary: To provide an efficient, effective, and quality community phlebotomy service, performing venepuncture on adult patients based in the community setting, at times in clinic ward / settings but predominantly for patients in their homes where the health and wellbeing of the individual would be compromised by travel. Job Purpose: The post holder will be required to perform a range of venepuncture techniques in the community setting and will work without direct supervision. This will require the post holder to undertake venepuncture on adult patients ensuring all relevant policy, protocols, Phlebotomy, and other relevant Standard Operating Procedures (SOP’s) are followed. The post holder will be a role model for phlebotomy and will provide support for newly appointed community nursing and health care assistants with phlebotomy skills.

Underpinning this philosophy is a Values Framework using the mnemonic ‘PATIENT’ with the expectation that all Procare employees demonstrate behaviours both during the recruitment and selection process and every day at work against each area in the Values Framework Procare ‘PATIENT’ Values Framework: Person centred care with a client centred focus Achieves clinical excellence, and inspires others Teamwork, working together in an integrated environment because everybody matters Integrity, in all that we do Experiences, are used for quality improvement and continuous learning New ideas, for clinically and cost-effective care Takes care of self, our own personal wellbeing is paramount KEY RESPONSIBILITIES: Communication / IT Communicates effectively with patients, carers, and colleagues maintaining positive working relationships Maintains a close working relationship with the clinical laboratory, wards, departments, and all users of the community phlebotomy service Ensures that patients are always treated safely and courteously and experience the minimum of distress or discomfort throughout the blood sampling procedure Uses communication skills to take blood samples from a wide range of patients, including those with mental or physical impairment or where there may be language barriers; and where information may be sensitive Performs routine venepuncture and reports any changes in the condition of the patient to the health professional requesting the venepuncture, both on the clinical IT system and verbally if urgent Provides constructive feedback to others undertaking venepuncture where tact and persuasive skills may be required Able to use electronic communication and keep accurate patients records in line with policy and procedures Uses clinical IT systems for the recording, monitoring and compilation of training, audits and service records as required Implements, monitors, maintains and records agreed standards of care Records demographics and daily activity data on the clinical IT system Maintains an up to date patient database Encourages feedback from colleagues, patients, and carers Quality / Safety / Patient Care Provides regular phlebotomy service to community hospitals, clinic settings and to patients within their own home communicating with colleagues, other employees, and GP’s Reviews phlebotomy referrals and prioritises visits according to clinical need under the direction of and supported by the District Nurse, ensuring that correct and accurate phlebotomy samples taken Checks accurate data on GP / hospitals phlebotomy request forms Decides most appropriate device and site to use to obtain a quality sample Able to assess patient to decide best vein to perform venepuncture Maintains specimen integrity by using aseptic technique, following all relevant policy and procedures Ensures that the correct blood sample is taken and patients who have not been bled are reported according to current procedure Able to take, label and transport correct specimens for requested blood tests in a safe and competent manner, ensuring that all blood samples are stored correctly and sent for testing within the correct timescale following policy and procedures Identifies and concerns regarding patient’s health and report in a timely manner to the lead nurse and / or GP Takes immediate action to ensure patient safety and follow up as required with other services as working as a lone worker in patient’s home, this may require you to contact GP, District Nurse, and other services Aware of the procedure to be followed in cases of Fire, Cardiac Arrest, collapse, and any other emergency.

Ensures all untoward incidents, accidents and areas of clinical risk are identified and reported to the lead nurse and on the electronic incident reporting system in accordance with policy Able to reallocate work as required in with community phlebotomy pathway Development of Self and Others Able to demonstrate correct technique for venepuncture and use of correct specimen and understands when to seek further direction from other health care professionals Keeps up to date with job knowledge by participating in educational opportunities, reading professional publications, and maintaining training portfolio Maintains knowledge and compliance with policies and procedures Works within Lone Worker Policy and reports when in / off duty as per service requirements Works unsupervised and be accountable for own actions while providing direct patient care Actively participates in internal and external training as appropriate to personal and team skills development Develops skills to undertake other duties as required by the service and consistent with the responsibilities of the role e.g. Blood Pressure monitoring, urine sample collection and equipment delivery Contributes to the development, implementation and review of community phlebotomy referral pathways as requested Responsible for ensuring mandatory training is kept up to date according to legislation and to local policies Ensures that own annual appraisal is completed, and objectives are set each year Works unsupervised and is accountable for own actions, whilst maintaining responsibility for allocated work Has responsibility for own schedule of work which may involve frequent changes at short notice to accommodate urgent referrals and patient / clinical need Ensures adequate stock levels for venepuncture, and is aware of budgetary issues and the need for effective use of resources Ability to work flexibly to provide an innovative service to meet the needs of the patients Participates in the orientation and induction of new staff and students pre and post registration members of staff Supports new employees in the adult community nursing service with phlebotomy skills Provides feedback to staff new to phlebotomy, supervising and supporting clinical skills Offers and supports a leaning environment to support new staff in learning phlebotomy skills
