Job Summary The pharmacy technicians will play an important role within the PCN, complementing clinical pharmacists and other members of the PCN multi-disciplinary team. Whilst this is a non-prescribing role, the role will work under supervision to ensure effective and efficient use of medicines and support patient care. The purpose of the role is to support the Primary Care Network to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes. The post holder will support the clinical pharmacist with Structured Medication Reviews.

The post will also help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. Pharmacy technicians core role responsibilities will cover clinical, and technical and administrative categories. The role will also include training and development of administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes. In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments.

This will involve assisting the PCN in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives. Applicants should be currently registered with the GPHC, have proof of CPD, be enrolled in or qualified from an approved training pathway-like the PCPEP Applicants should also have: A full UK driving License or suitable access to transport to be able to travel to sites within the PCN area DBS check (will be conducted by Imp upon job offer) Job Responsibilities Clinical/Patient Facing Take a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients. Support the Clinical Pharmacist in Structure Medication Reviews (SMR) e.g. organise necessary monitoring tests prior to SMR To undertake medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly.

To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols. To participate in medication audits. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing; To support the achievement of prescribing Quality and Outcomes Framework targets, QIPP, Improvement and Innovation Fund and other commissioned services across practices and the PCN. To provide advice to GPs, staff and patients in changes to prescribing to support the improvement of prescribing safety, quality and cost effectiveness.

To assist in the appropriate monitoring and management of prescribing budgets and to liaise with the medicines management team at the CCG. To prepare evidence based resources and information to support the implementation of rational cost-effective prescribing. To support the Practices to provide medication reviews, patient information leaflets, medicine awareness; and to undertake community visits as appropriate. To take a proactive approach to the safe handling of prescriptions, including assessment of risk and making recommendations for improvement.

To advise on the sourcing and safe management of medicines as appropriate. Administrative Responsibilities Other duties and responsibilities to be undertaken may include any or all the items in the following list: Implement efficient ordering and return processes and reducing medication wastage. Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS). Promotion of Electronic Repeat Dispensing (eRD) and online ordering Working with the Practices to contribute to the review and development of repeat prescribing protocols / Standard Operating Procedures to improve the efficiency and effectiveness of repeat prescribing systems.

Support the implementation of national prescribing policies and guidance within GP practices and other primary care settings. Taking messages and passing on information as appropriate. Maintain SystmOne templates to ensure accurate and detailed clinical data is entered into the patient record in a standardised way. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required in accordance with practice procedures.

Provide comprehensive outcome focused reports detailing the progress Initiating contact with and responding to requests from patients, other team members and associated healthcare professionals and providers. Helping to maintain a positive, patient focused and supportive culture. Any other duties commensurate with this position. Other Responsibilities To assist the PCN Director(s) and board in setting and realising the PCN vision, mission and business strategy To play a role in the delivery of high-quality primary health care services Provide support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.

Working under supervision of the Clinical Pharmacists as well as from core network member practices (when undertaking practice specific duties) i.e. the GPs, Practice Managers and the Business Manager take referrals from the network practices. Maintain a working relationship with local health care providers enable service delivery of mutual benefit and build a network and knowledge of referral routes to and from service providers. To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary Refer people and/or introduce them to appropriate organisations in Lincoln and nationally (where appropriate) e.g.

voluntary, statutory (local authority) and local NHS organisations. To represent the PCN in cross organisation meetings when agreed e.g. locality/CCG meetings Collate feedback / analysis data on behalf of the PCN to report to the PCN Board and CCG/NHSE as required Be responsible for the organisation, planning and of own workload to meet set deadlines. Following PCN and practice policies and procedures as appropriate.

To inform and shape the role to an ever changing environment Duties will vary from time to time under the direction of the Clinical Directors dependent on current and evolving PCN workload and staffing levels. All Pharmacy Technicians must work within their competencies, and have professional indemnity for their role.
