This is an innovative award winning service delivering new models of care within WLMHT.

The positions are 1 full-time Social Worker working in the Ealing Perinatal Service.

The fulltime post will be based primarily at St. Bernard’s Hospital where our Ealing Perinatal team are based but will work on the Tri-Borough duty rota providing rota cover to Queen Charlotte’s and Chelsea Hospital and Queen Mary’s Maternity unit.

This would be an ideal opportunity for an excellent well experience Band 6/Grade 10 and above Social worker with excellent assessment skills to work in an area and a developmental role in a specialist field.

We would support the successful candidate to develop a bespoke training programme to facilitate their development within the post.

Main task and duties includes:

  • Ensure that women (regardless of age) who are pregnant and booked for delivery and/or have infants up to a year old receive appropriate screening, assessment and identification of their mental health needs and are these are managed using evidence based interventions throughout this period
  • Actively liaise with primary care, maternity and health visiting staff as well as other mental health services regarding referrals, care co-ordination, transfer of care including facilitating professionals’ meetings
  • Efficiently manage caseload of up to 25 (Fulltime)/15 (part time) women (antenatal and postnatal) with complex mental health needs, by providing assessments and reviews in community sites (children centres), joint reviews with maternity, health visitor or other mental health professionals, home visits and care planning meetings
