Batley Birstall Primary Care Network is a patient-focused forward-thinking PCN looking for people to work with the PCN team to deliver care to their patient populations. The Care Coordinator role is a new and evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work, the role will consist of some focused project work as well as general core support to the PCN. The post holder would ideally have good knowledge of the community or primary care sector and strong IT skills( knowledge of Systmone is desirable) the role will be key to practice searches and audits.

Responsibilities
Care Coordinators provide extra time, capacity, and expertise to support patients in preparing for or following-up clinical conversations they have with primary care professionals. They will support the Clinical directors in coordinating all key activities including improving access to services, providing advice and information and ensuring health and care planning is timely, efficient and patient-centred. They will work closely and in partnership with the Social Prescribing Link Workers and support the delivery of the comprehensive model of care to reflect local priorities, health inequalities or population health management.
Key role requirements Care coordinators will:
Running reports on Systmone and using the data to identify workload and population groups that require action and support.
The Care Coordinator will support the achievement of the practice of s PCN and Quality and Outcomes Framework initiatives. To audit and maintain an overview of clinical achievement within the practice and ensure the efficient and timely recall of patients for clinical review ensuring coordination of patient care.
The applicant will work with the practice teams to enable the achievement of practice targets, national directives, policies and priorities. The role will include supporting digital initiatives and responsibilities for coordinating the patient's journey through primary care.
Help maintain a positive, supportive patient-focused culture across the practice team.
Participate in training programmes as agreed.
Provide coordination and navigation for people and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches and other Primary Care roles.
Liaise with Support workers, GPs Social Prescribing Link Workers and other healthcare professionals to identify patients that would benefit from one-to-one support and provide this using the available decision support aids.
Feedback appropriate patient information to practices to ensure all services can support the timely care of the patient.
Help patients manage their needs and support them to make appointments
Raise awareness of shared decision-making and decision support tools and assist people to be more prepared to have a shared decision-making conversation.
Ensure people have good quality information to help the make choices about their care.
Assist people to access self-management education courses, and peer support interventions that support them in their health and wellbeing.
Specific project work liaising with practice leads to the implementation of the shared process, and collaboration opportunities to support improved patient care.
Creating, modifying and deploying Excel reports in line with Business Requirements.
Collaborative working and building relationships across varied organisations
Desirable
Confident in dealing directly with patients/professionals
Comprehensive understanding of patient confidentiality
Experience in following legal, ethical, professional and organisational policies/procedures and codes of conduct
Experience in working within general practice/primary care
Experience in clinical IT systems/reporting/audit
Understanding of medical terminology
Qualifications
Essential
Demonstrable commitment to professional and personal development
Desirable
NVQ Level 3, Advanced level or equivalent qualifications or working towards
Training in motivational coaching and interviewing or equivalent experience
Systmone coding

Salary: £21,750.00 per year
