Your role
The post holder will work as a key member of the Pharmaxo Healthcare Quality Team, to support and deliver a range of quality governance activities across our Pharmacy, Patient Services and clinical teams. They will be a lead in ensuring a patient safety culture is maintained and ensuing regulatory compliance. They will manage the Pharmacovigilance team to ensure compliance with reporting obligations and provide mentorship to staff on understanding the importance of maintaining a patient safety culture.
The post holder will have a pivotal role in developing and managing the systems and processes in relation to all patient safety incidents; including complaints and incidents, adverse drug events and all other safety data. They will work collaboratively with operational teams to improve reporting, comprehensive investigation and learning from all safety incidents.
The post holder will work closely with the Clinical Governance Lead to ensure patient risks are managed in line with the Royal Pharmaceutical Society Homecare Standards and other regulatory requirements. They will ensure strategic development of Policy and Standard Operating Procedures to demonstrate compliance with GPhC, Care Regulators and MHRA requirements.
The post holder will lead on the production of comprehensive governance and safety reports and facilitating meetings with key stakeholders to support governance processes. The post holder will be a subject matter expert in inspections including Care Regulators, General Pharmaceutical Council and Supplier Pharmacovigilance audits.
Your duties & responsibilities
- Support the Pharmacy Manager, Head of Nursing and Head of Quality with developing quality governance frameworks, demonstrating effective communication with services and frontline staff on the wider range of patient safety activities.
- You will ensure an open incident reporting culture and be active in encouraging staff to report incidents.
- Supporting the Quality Team and Service leads by collating evidence for governance action plans that have been implemented.
- You will support the Head of Quality in the management of the Quality Risk Management program to ensure patient safety risks are identified, documented and controlled.
- You will be the lead for complaints and incident management, ensuring all investigations are completed in a timely manner and developing root cause investigation skills and expertise within the wider team.
- You will develop a risk based approach to investigation of incidents to ensure effective processes are in place.
- Monitor the systems in place to ensure tracking of clinical incidents, investigations and action plans and report trends to appropriate department leads as required.
- Act as support to any investigation team established under the serious incident procedure for all incidents both serious and non-serious working alongside the Clinical Governance Lead as required.
- You will act as Information Governance Lead, developing training and governance procedures that ensure regulatory compliance.
- In partnership with key stakeholders, lead on the development of relevant policy and Standard Operating Procedures (SOP). Ensuring these are affectively designed, maintained, and communicated to all relevant individuals.
- Be responsible for improving the safety reporting quality systems and processes.
- Lead on Patient safety audits and ensure effective feedback of the outcomes, learning and solutions where possible.
- Develop and implement strategies to promote good governance and effective risk management across all roles.
- Initiate and lead projects to deliver organisational governance management requirements, applying sound project and change management practices.
- Oversee quality improvement projects; producing high level reports and provide robust governance expertise to projects and services.
- Carry out annual reviews of each service to ensure processes, SOPs and policies are being followed. Provide a report on recommended improvements for service managers to deliver.
- You will represent the quality team at customer meetings relating to incident management, patient safety reporting and service quality.
- You will work closely with the Quality Coordinator for Pharmacovigilance to ensure all Individual Safety Case Reports (ISCRs) are reported within agreed timeframes.
- You will oversee Good Pharmacovigilance Practice and ensure systems and processes are effective.
- You will be accountable for Pharmaxo KPI performance for all PV activities.
- You will contribute to the preparation of accurate, high-quality pharmacovigilance and safety reports within agreed regulatory timelines to assist in meeting the Companys development targets and excellence.
- You will be required to develop and deliver mandatory quality training on patient safety activities for all staff, including management of complaints and incidents in line with RPS Homecare Standards.
