Job description
This is a varied administrative role, in which you will form part of our focused patient call centre and GP referral team, providing high quality, accurate and timely referral processing. Our patient support Support Service helps patients to receive the most appropriate treatment and also allows them to make an informed decision about where they would like to attend for treatment. Additional to this you will need a clear understanding of all pathways the service manages as well as the transfer process with waiting list management a current focus. You will also be required to learn, understand and support the Covid Vaccination team, which could include training and development and mentorship. As the RSS is ever evolving and fast paced you must also be adaptable to change, as a service we are here to provide care and support not only to patients but also to services that need our support.
To be self led, motivated and able to report on daily tasks, achievements & challenges faced working autonomously in a home environment.
To answer all calls coming through the Referral Support Service and to make out-going calls to support in waiting list management and patient care.
To be able to review Referrals and ensure all relevant information is present for a triage decision to be made.
To analyse and review patient information to ensure patient is suitable for referral as well as suitable for the care settings when sending patient choice letters.
To be able to arrange, cancel or amend Primary Care appointments, via System One and other NHS computer systems, in line with Government and Trust guidelines.
To show humility, understanding and care when contacting patients deemed suitable for transfer of care. To facilitate and understand the settings and providers of transferred care.
Communication and liaison with GP surgery’s, Primary Care Providers, Secondary Care departments and Triage GPsWI’s is a key part to this role and will require an in-depth knowledge of the patient access policy and all other hospital policies and procedures.
To support in ad-hoc patient care plans by data gathering and effective entry for clinical decisions to be made or transferred.
To support the Covid Vaccination Booking team when required. This will involve additional training and acting as a mentor and trainer to temporary members of staff when required. There may also be the requirement to support and call patients yourself should large volume of patients need contacting.
Due to training and nature of calls, living in or near to Leicestershire would be an advantage but not essential.
To undertake any other duties in line with the grade as directed by the Team Leader.To be flexible and welcome change, the RSS is a dynamic and forward thinking team. You need to be able to think on your feet, learn processes quickly and action tasks as they come in order to support reduction in waiting lists and ensure the patient gets the best possible care in the best setting.
