About Liaison Care
Liaison Care is at the heart of supporting the NHS standardise and improve the end-to-end delivery of CHC. This ensures that ICSs can guarantee everyone has fair access to CHC and can ensure that patients and families have better experiences and outcomes, whilst helping the NHS make better use of its resources in this area.

Liaison Care
has developed a range of products and services to support ICSs transform and manage the challenges of delivering CHC. These include transformation reviews, expert advice and software to help accurately predict personal health budgets.
We are currently working with some prestigious NHS Organisations to help them to reduce their backlog of cases and reviews.

The team
You will work alongside other Administrators and Officers. The wider team is all based remotely across the UK, with a lot of NHS experience and knowledge utilised across the function. They are fun and driven towards finding innovative ways to help vulnerable individuals.

You would be based at home, with full access to equipment including IT programmes such as Microsoft Teams. There will also be the ability to meet with others in the team where required in order to provide you with full support.

You must be able to travel to team meetings where required.

Main responsibilities
The purpose of this post is to provide a comprehensive administrative service within the Care Division. The post holder’s main duties will include planning, organising, directing, and coordinating the work of the department.

The Pathway Coordinator will be expected to work with minimum supervision. You will be able to demonstrate flexibility and use your own initiative to carry out duties in such a way as to make a direct and positive contribution to the organisation, thus enabling the CHC Practitioners to utilise their time in an efficient and effective manner.

You will provide a confident professional service to all individuals and their representatives, whilst supporting them throughout their pathway of care. It is vital that confidentiality be maintained at all times.

Experience and qualifications required
You will have worked as a Coordinator or Administrator in another capacity, using computerised systems and dealing directly with customer queries.

It is not essential; however, we are ideally seeking previous CHC administrative experience.

You must be able to demonstrate the ability to organise own duties and prioritise tasks within a busy team environment and to work without direct supervision.

You will hold excellent communication skills, both verbally and non-verbally when dealing with patients/relatives and other internal and external agencies especially when providing or receiving complex or sensitive information.

You will also have the ability to demonstrate experience within information systems, e.g., word processing packages, spreadsheets, databases, patient administration system, email etc.
