1.1 Job Summary

Bexley Health Neighbourhood Care (BHNC) is a GP Federation supporting the 22 Bexley Practices / 4 PCNs who are working collaboratively to enhance the health and wellbeing of Bexley residents, covering over 246,000 patients.

As a phlebotomist, the job holder will work closely with the GP practices, ensuring all patients across the locality receive the best possible care and service. This role will support the GP Practices with urgently requested blood diagnostics. Working strictly in accordance with guidelines and protocols, the post-holder will be responsible for performing venepuncture to obtain blood samples from patients only as the result of a direct referral from a GP or nurse practitioner.

The job holder will ensure the seamless coordination of the patient’s journey through primary care and will build trusting relationships with the GP surgeries within their PCN. Creating coordinated patient care, enabling the individual to receive timely access to facilities.

Bexley Health Neighbourhood Care (BHNC) aims to improve the morale of PCNs / general practice in Bexley, by sharing expertise, services and supporting its workforce. BHNC will make a positive impact on medical services in Bexley, by working closely with the CCG, local NHS trusts, local providers and patient groups, to improve the delivery of healthcare to the local population.

1.3 Key Responsibilities

Duties

· To work with GPs and other primary care professionals within the PCN to support patients to access urgent blood diagnostic requests and record on Vision system.

· Ensuring that the patient is as comfortable and relaxed as possible and confirming their agreement to the collection of a blood sample

· Collection of sample(s)s of blood in accordance with training and procedures

· Ensuring samples are stored in appropriate containers

· Accurate, legible labelling of all samples to confirm patient identity

· Checking patient identification against GP/nurse request and confirming with patient whenever possible

· Recording collection of blood sample in patient record in accordance with procedure

· Ensuring that samples are dispatched to the appropriate laboratories in a timely fashion.

· Ensuring that anomalies are reported to a senior member of staff

· Notifying the GP/practice staff of any significant patient issues

· Liaising with and assisting members of the administrative team

· Work effectively within the phlebotomy team and alongside the reception/admin team

· Report any clinical or environmental issues of concern and take action to rectify these where appropriate

· Ensure a working knowledge and compliance with Policies and Procedures

· Understand the importance and adhere to infection control issues

· Keep clinical areas clean and free from clutter.

· Input information into the clinical system as appropriate following appropriate training.

· Demonstrate a working knowledge of the Policy on information governance.

· Handle confidential patient material in a sensitive and discrete fashion in compliance with Bexley Health Neighbourhood Care policy and procedure.

· To be able to influence, motivate and involve individuals and teams to reach necessary performance target expectations.

· Liaise with other departments and members of the Bexley Health Neighbourhood Care and other organisations to ensure the smooth running of the role to assisting the functioning of the PCN team.

· Work within BHNC policies and procedures.
