SHOREDITCH PARK & CITY PCN
Job Title: PCN Paramedic Practitioner – Band 7
Responsible to: Clinical Director
Accountable to: Clinical Director
Hours of work: 37.5 hours per week
Salary: £47,500 to £52,000
Fixed Term Contract: 18 months
City and Hackney has eight Primary Care Networks (PCNs) working in partnership. Paramedic roles across the differing PCNs will not only work within their PCN team but as a team of their own to allow for consistency in approach and service delivery.
Shoreditch Park & City PCN are an NHS collaboration between 5 GP Practices in the Shoreditch Park and City areas of Hackney. Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan. They provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve. PCNs will help to join up services at a local level, focusing on the specific needs of these local populations. Our aim is to give our patients better access to the right care and support, at the right time.
PCN Paramedics work autonomously within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. They are health professionals who practice at an advanced level having the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.
The Shoreditch Park & City PCN network is seeking a fully experienced Paramedic Practitioner to join our team. You will be working across the PCN and no two days will be the same, allowing for a varied and fulfilling role. The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.
Key duties and responsibilities
- assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN team
- advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
- be able to: perform specialist health checks and reviews
- perform and interpret ECGs; alongside other results as appropriate
- perform investigatory procedures as required
- undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.
- perform investigatory procedures needed by patients and those requested by GPs and the wider PCN
- support the delivery of anticipatory care plans and lead certain community services (e.g., monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
- provide an alternative model to urgent and same day home visits for the PCN and undertake clinical audits
- communicate at all levels across PCNs and other organizations, ensuring effective, patient-centered service
- communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
- collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
- maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
- prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- work with patients in order to support compliance with and adherence to prescribed treatments
- provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs
- clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- prioritize health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
- Be flexible and available in their daily working hours, enabling some hours to be covered and worked during Enhanced Access clinics.

- assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
- participate in research and utilize the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
- in partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate
Administrative requirements
- produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies and procedures
- be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
- use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information
- review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
Supervision
The postholder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day-to-day basis.

Proof Stage 1 of the HEE Roadmap to FCP has been completed with a portfolio of evidence and verified.

Commit to obtaining Stage 2 accreditation and portfolio of evidence within Primary Care. It is expected that a trainee FCP will achieve their level 7 clinical practice pillar within 6 months of being recruited into Primary Care.

Already completed, undertaking, or prepared to undertake a Level 7 FCP Module approved by the HEE Centre for Advancing Practice which includes Stages 1 and 2.

Independent Non-Medical Prescribing
