Job summary
Our Health Partnership are seeking an experienced paramedic who will be able to transition their ability to deliver high quality pre-hospital care into the general practice environment.
The role will involve conducting telephone assessments, face to face consultations and home visits across practices within the Kingstanding, Erdington & Nechells Primary Care Network (KEN PCN).
This is an exciting opportunity to join a dynamic multidisciplinary team including Pharmacists, Nurses, Doctors, Health Care Assistants, Physician Associates, Social Prescribers and Physiotherapist to name a few. You will be supported by the multidisciplinary team (MDT) including Paramedics and General Practitioners to develop and expand your clinical and diagnostic skills as well as fully funded training, various CPD opportunities and support from a Lead Paramedic, Paramedic peer review network and local primary care training hub.

Main duties of the job
The role will involve working as an autonomous, accountable Paramedic in a general practice setting. You will be required to assess, diagnose, and treat a variety of presentations incorporating same day emergencies, minor illness/injury, and management of long-term conditions. We are looking for individuals who will be able to utilise or develop advanced clinical assessment skills, provide a holistic approach to patient care, and empower patients to make informed choices about their treatment.
Please note: The post holder will be a minimum of five years post qualification at the time of employment and will be required to complete or have completed First Contact Practitioner level 7 university module. You will be supported in practice to achieve this qualification by our Doctor and allied healthcare professional team and will be a fully funded level 7 university qualification.
Please only apply for this position if you are a Qualified and experienced Paramedic with five years post registration. Applications from paramedics without five years post registration experience or other health care professionals will not be considered.

Job responsibilities
The role will provide a specialist Paramedic resource to support our patients whilst working in collaboration with other members of the Primary Care Network (PCN) and Multidisciplinary Team (MDT).
The post holder will be required to work in GP Practices within the local PCN and be responsible for assessing, diagnosing, and treating undiagnosed and undifferentiated conditions across all age ranges. Patient assessment will be conducted via telephone, video, messaging, face to face and home visit consultations. The post holder should focus on hospital avoidance and be able to recognise the need for referral or signposting of service users to alternative care pathways where appropriate. There may be a need to engage with a palliative care or care home caseloads as part of a multidisciplinary team, which would involve the assessment and treatment of acute and chronic presentations and engaging patients and their relatives in discussions about advanced care planning.
Full training will be provided to support you to develop and maintain your clinical skills and scope of practice. As part of your role, you will be able to understand and record specific patient targets to ensure that the high levels of the care you provide are recorded and reflected across the PCN.

Experience
Essential
- The ability to evaluate a patients body systems in a methodical, thorough manner while using the medical model. A clinical diagnosis and therapy strategy are developed by interpreting clinical symptoms in conjunction with diagnostics. The capacity to recognise ones own clinical practices evolving scope and, when necessary, escalate to a senior practitioner.
- Practical and theoretical caseload management experience, including completion of assessments, planning, implementation, and evaluation of interventions that are in line with care plans.
- Working understanding of the principles and values of the NHS, HCPC, and College of Paramedics.
- Ability to write comprehensive clinical notes, implement and evaluate care plans and to formally handover to other healthcare professionals.
- A demonstrable portfolio or record of ongoing career development (CPD)
Desirable
- Working towards Health Education Englands Advanced Clinical Practitioner status
- Mentorship or supervisory skills training
- Leadership training or experience
- Awareness of data protection (GDPR) and patient confidentiality
- Minor illness or injury assessment and management

Qualifications
Essential
- BSc or working towards a BSc in a training programme approved by the College of Paramedics or equivalent evidenced learning.
- Health & Care Professions Council (HCPC) registration as a Paramedic with no restriction to practice.
- 5 years post registration as a HCPC Paramedic
- Ability to operate or work towards an advanced level of clinical practice.
- Driving License and access to a vehicle
- Evidence of level 6 study working towards BSc.
Desirable
- Membership of the College of Paramedics
- Basic or immediate life support trainer
- Non medical prescriber
- Emergency Care Practitioner, minor injury/illness at level 6 or 7 qualification
- Leadership, mentorship, coaching or management qualifications
