Salary: £27,882 - £29,120 depending on experience (pro-rata if part time)
Hours: 35 hours per week (part time applicants may be considered)
Days and times: Monday - Friday, 9am - 5pm (or specific days if part time)
Contract: Permanent
Responsible to: Information & Advice Manager
Location: Flexible across Borough of Hillingdon to support delivery

We are seeking an experienced advisor to provide specialist information and advice relating to welfare benefits and housing to older people with mental health issues and dementia, on Oaktree Ward Hillingdon Hospital and within the community, following discharge.
Working with partner organisations to focus on supporting discharge in particular the mental health team, reducing risks of readmission, and tackling some of the wider health determinants that impact wellbeing.
