An exciting opportunity has arisen to be part of the Complex Recovery team. This role includes working under the band 7 and 6 O.T.'s to work practically with those individuals that have recently returned from out of area rehabilitation placements, living across the four localities within the Black Country. These individuals will be open to secondary care services with a diagnosed Severe Mental Illness and requiring community rehabilitation support.
The post holder will also be responsible for supporting the assessment and intervention plans of the senior Occupational Therapists to support the complex mental health needs of individuals, ensuring that the placement and or package of care is appropriate to their needs and step down to locality based services. The post holder will work intensely with the client for 12 months, focusing on helping to build resilience, improve their wellbeing and tailoring interventions to meet their individual, cultural and social needs.
Main duties of the job
The post holder will work as part of the Complex Recovery Rehabilitation Service MDT, which includes a Consultant Psychiatrist, Psychologist, CPN's and Occupational Therapists. The post holder will liaise with external agencies to achieve personalised recovery outcomes for patients within the service.
The post holder will be able operate flexibly from home and /or a community-based facility and will provide service from the work base, via the telephone or within the service user’s place of residence. Some travel will be required once the team go live across the four areas within the trust.
The service will operate Monday to Friday 9-5 hours but is expected to advise service users and carers of access to services outside of these hours and liaise and refer on to other mental health care providers in cases where support outside of these hours is required.
