You will be key support for service users in achieving increased independence, working to promote their wellbeing and daily living skills by teaching, assisting in personal care and motivating people in re-learning activities as part of the outcome based Reablement plans designed by Occupational Therapists. What you'll be doing:
- Explaining to residents the process of Integrated care, including how the resident will be supported but also discharged from the service. Arranging for the delivery of services to support the resident in their home, to liaise with Reablement, locality teams, equipment providers, housing services and the health hub, paying due regard to any statutory responsibilities.
- Evaluating this role in term of the patient outcomes, carers views, operational outcomes and use R- outcomes.
- Carrying out complex assessments of patients’ needs and decide how to meet the identified social, physical and psychological needs of residents. To provide this assessment in the residents own homes using all available resources, including the use of videoconferencing facilities, if required.
What you'll have:
- Demonstrable experience required to work autonomously in an integrated MDT primary care health and social care setting.
- Ability to manage autonomously a clinical caseload of clients at a specialist level, assessing and treating clients and maintaining clinical records.
- Recent knowledge of falls management and education, rehabilitation programs, frailty and complex care management and good general knowledge of the safe installation and use of assistive equipment and adaptations.
- Excellent time management skills. Including the ability to work to deadlines
- Positive attitudes and behaviours, taking a strength based approach to assessments
- Experience of working with people of diverse cultural and religious communities
- MSC/ Bsc/Diploma in Occupational therapy
- State Registration (HCPC & College of OT)
- Full Driving Licence with access to a car on a daily basis
