Be part of a team which has a Reablement service which, last year, helped hundreds of people become independent again after a stay in hospital, where Occupational Therapists, domiciliary rehab staff and social workers find innovative ways to support people to live how they like using direct payments, where an incredible partnership with health means integrated teams which deliver services more effectively.
You will be key support for service users in achieving increased independence, working to promote their wellbeing and daily living skills by teaching, assisting in personal care and motivating people in re-learning activities as part of the outcome based Reablement plans designed by Occupational Therapists. You will be making a true difference to people’s lives!

What you’ll be doing:
- To work as a member of a community based multidisciplinary team.
- Use Occupational therapy expertise to assess, implement, guide care plans for service users and carers, enabling them to achieve maximum independence in their own homes or residential settings.
- Provide a broad range of support to clients who have physical or mental health problems, learning difficulties, dependencies or who are elderly.
- Help to develop and/or maintain resident’s independence using a strengths based approach.
- Requirement to work on a shift basis including evenings and weekends
What you’ll have:
- Ability to manage autonomously a clinical caseload of clients at a specialist level, assessing and maintaining clinical records.
- To set SMART goals to ensure treatment is effective, and to improve client flow through the service.
- To work collaboratively with health and social care community and acute colleagues.
- To work with other members of the MDT to ensure effective assessment, treatment, management of care pathways and transfers of care / discharges.
- Excellent time management skills. Including the ability to work to deadlines and in stressful situations
- Demonstrate positive attitudes and behaviours, taking a strength based approach to assessments
- Experience of working with people of diverse cultural and religious communities
- MSC/ Bsc/Diploma in Occupational therapy
- State Registration (HSPC & College of OT)
- Full Driving Licence with access to a car on a daily basis
