You will join our dynamic Multi-Disciplinary Team of who strive to understand what matters to our patients; enabling patient centred care to achieve their goals. Your support will enable our patients to understand more about what has caused their health decline and engage them in meaningful rehabilitation that encourages self-management impacting on long term outcomes.

At present the Planned Community Therapy service operates from Monday to Friday core hours, however the job description reflects a seven day service in case of changes to service need.

To be an integral part of the Out of Hospital Service.

To effectively manage a caseload of patients as part of a multudisciplinary team (MDT), providing specialist and evidence based assessment and interventions to patients in both a stable and acute phase of their condition, ensuring the physical, psychosocial, behavioral, perceptual, cognitive, sensory and environmental needs of patients, are addressed.

Utilising specialist knowledge and clinical reasoning, to plan and provide clinical interventions in consultation with other colleagues in the multidisciplinary team and to work in a range of settings as appropriate (e.g. patients home, work environment or other community setting)

To adopt a patient centred and sensitive approach to therapy, collaborating with patients (and/or carer(s) where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting)

To utilise individual and/or group work skills as appropriate in order to achieve agreed therapeutic goals.

To have a high level of knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls and specialist knowledge of at least one of these clinical areas
To work collaboratively with Social Services and third sector providers to ensure effective safe care.

To provide specialist occupational therapy assessment, treatment and review of patients with a wide variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls, to reduce unnecessary hospital admissions, contribute to effective admission avoidance and support patients with complex health care needs. This will be for patients predominantly in their place of residence, but will include clinic and other settings.

To provide advice and guidance to others, patients, carers and staff working in health and social care.

To support professional development of the staff within the Out of Hospital Service and other services as appropriate.

To support and contribute toward service development and meeting Key Performance Indicators.
