How you'll make a difference

South Gloucestershire Council are pleased to be partners in the development of the North Bristol Trust (NBT) Transfer of Care Hub (ToCH) and to assist in the co-ordination of safe discharges through a trusted integrated whole system approach. The ToCH will bring together community partners (Sirona), Local Authorities and Voluntary Sector Colleagues across BNSSG to create a new team with multidisciplinary backgrounds. The Hub team will include Acute Trust Case Managers, and Patient Flow Coordinators, Local Authority Discharge Coordinators, Sirona Case Managers, Community OT's, Commissioners, Social Care Practioners, Voluntary Services and Flow and Discharge Coordinators.

The ToCH Team will play a key role in aiding discharge and admission avoidance. The aim is to work collaboratively and holistically, striving to create a person centred service to maximise the opportunity to discharge people at the point they are ready to leave hospital. You would be integral in providing an excellent service to our patients easing their transition from hospital and ensuring that hospital beds are available for people that need them.

You will make a difference as an Occupational Therapist/Advanced Occupational Therapist in our Council with passionate service delivery, working alongside a highly supportive and committed team, who work with people over the age of 18 in supporting them to live at home by promoting their independence. You will focus on ensuring that you contribute a positive approach in supporting and meeting the needs of our residents in the community.

Salary are offered at points within these bands depending on experience:

  • Occupational Therapist: £32,020 - £34,723
  • Advanced Occupational Therapist: £36,298 – £39,493

What will you be doing?

Our Adult Social Teams consisting of Occupational Therapists, Social Care Practitioners (OT) and Social Work Practitioners. You will join our Occupational Therapy team, with prevention as its focus, enabling you to grow your skills in the varying and different areas of Occupational Therapy including:

  • Adaptations
  • Equipment (standard and specialist)
  • Moving and Handling
  • Rehousing
  • Reablement
  • Multi-disciplinary working with Health Colleagues
  • Joining our community team you will visit and discuss with our service users and carers, their needs in relation to their home environment, identify with them their priorities and goals in order to assist them and their families and carers in planning realistically for the future. Working closely with the service user and relevant others you will then formulate and implement a support plan.
  • You will be allocated a caseload with support and regular supervision, where following assessment, as appropriate arrange for the provision of equipment, instruction in its safe use. To carry out risk assessments with service users empowering them to reach their own potential rather than restrict what they can do.
  • You will advise and inform Disabled People and their Carers about Services available, including services available under the Care Act. You will work within the 3 Conversations Model, utilising a strength-based approach.
  • Be responsible to make important recommendations for a significant budget spend ensuring maximum value is delivered for the resources deployed.
  • The role will require you to be a skilled communicator, including liaising and work with contacts from different sectors, including Health and voluntary.

As an Advanced Occupational Therapist, you will be required to:

  • support students on placement as an experience practitioner
  • be fully up to date with the core training expectations as an Advanced Occupational Therapist
  • examples of using research and evidence-based resources to enhance practice and share learning with team (e.g. use of Research in Practice / other online resources).



What we need from you

  • It is essential that you have a Professional Occupational Therapy Qualification, Dip COT, BSc Honours OT, (or equivalent), and demonstrate current State Registration and be registered with the Health Professionals Council

  • In the role you will be able to demonstrate how your experience of working in a hospital or community setting or voluntary or agency providing Occupational Therapy Services is relevant to this post.

  • You must prove that you can demonstrate theoretical and applied knowledge relating to Disabled People and their needs.

  • You should show your up to date and sound knowledge of all related legislation relevant to Social Services Occupational Therapy and legislation supporting this

  • You will have experience of managing complex cases and demonstrate that you can apply excellent assessment skills in practice.

  • You must be excellent at time management so that you are able to work efficiently to meet the service requirements.

Advanced Occupational Therapist:

  • We will require you to have a minimum of 2 years post qualification experience.
  • You will have evidence of meeting the essential (and desirable where possible) criteria for the role of Advanced Occupational Therapist or in South Gloucestershire council.
  • A clear commitment to Continuous Professional Development (CPD)

You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle or have alternative appropriate means of travel.

Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post
