This vacancy is to work within the Community Integrated Assessment Team (CIAT) providing therapy intervention to those patients that require support to regain or maintain their independence. The CIAT aims to support timely hospital discharge and prevention of admission to hospital or a care setting.
It is expected that the successful candidate will work with the Clinical Team Leader for CIAT in planning, co-coordinating, delivering and evaluating service development initiatives. Along with being responsible for their own caseload and the supervision of post graduate therapists and therapy assistants within CIAT.
We are excited to offer the opportunity for motivated, enthusiastic Band 6 Occupational therapist or Physiotherapist to work within the CIAT services based within North tees and Hartlepool NHS Foundation Trust.
The post holder will be expected to work within the community therapy services across Stockton and Hartlepool. CIAT currently provides a seven day service and offers an extended service till 7pm Mon-Fri. The post holder will work within CIAT delivering therapy across the areas which may include, community therapy, urgent crisis response, and residential rehabilitation.
The post involves working in partnership with all stakeholders including partners in health and social care to ensure the provision of high quality care delivery to this group of patients and their carers.

Responsibilities of the role will include a comprehensive assessment to aid in the reablement of the patient either at home or in a rehabilitation unit. Attending to urgent crisis, assessing the patient working towards hospital avoidance and care closer to home.
Making every contact count to influence behaviour change and encourage health promotion, providing personalised care to empower patients to live well in the community.
To provide people who live in Stockton and Hartlepool timely and appropriate therapy interventions in partnership with the multi disciplinary and multi agency team and/or provide rehabilitation needs identification and liaison with appropriate Community Services
Assessment of people’s needs and the prescription/provision of appropriate
equipment/adaptations to enable their safe return to their lifestyle of choice.
To take a lead role in the advanced assessment and treatment of patients with complex and/or chronic presentation, and to determine clinical diagnosis and appropriate management plan, and to maintain records as an autonomous practitioner.
To treat patients in a variety of locations including community, residential/nursing homes and in the patients own home.
