An exciting opportunity has arisen within the Edinburgh Health and Social Care Partnership’s Home First service for two Allied Health Professionals, specifically Occupational Therapy and / or Physiotherapy to support the delivery and implementation of the Discharge without Delay Programme.
The Partnership has seen significant organisational change driven by the aim of improving outcomes for the citizens of Edinburgh who need our health and social care services. The Partnership has to rapidly adapt and build service models that improve outcomes for our citizens. A national initiative led by the Scottish Government to improve Urgent and Unscheduled Care has identified eight high impact areas which includes the Discharge without Delay programme.
The successful candidates will be proactive and self-motivated, with a working knowledge of locality services and hospital settings, which will require an understanding of community resources. They will provide direct operational support and line management to the Home First team, including operational governance and accountability.
The roles will be based within hospital settings at the Western General Hospital and Royal Infirmary of Edinburgh and will require on-site presence. The successful candidates will provide a link to community therapy teams and will work collaboratively with acute based therapy teams where applicable. These roles will evolve and develop which will require the successful candidates to adapt and support the development opportunities arising. Both posts will report directly to the Home First Service Manager.
