Job Title: Primary Care Occupational Therapist (OT)
Responsible to: Practice Manager
Accountable to: Partners and Practice Management
Hours of employment: up to 28 hours per week
Salary: Approx. equivalent to AFC Band 7 (not AFC contract)
Job Summary:
This post requires an OT who is highly specialised and can work at an advanced level of practice within the Linden Medical Group, utilising their in-depth knowledge, clinical skills and experience at a senior level, in the management of patient’s occupational therapy conditions.
The post holder will provide intervention for the identified cohort of patients which includes the frail, elderly, care homes, palliative and complex patients from initial clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for. They will work collaboratively with the Practice’s and
PCN’s multi-disciplinary team (MDT) to meet the needs of patients and carers and support the delivery of policy and procedures.
The post holder will lead a team of Care Coordinators providing guidance, line management and support.
Clinical Role:
- Deliver high quality patient care as a primary care occupational therapist using advanced autonomous clinical skills and a broad in-depth knowledge base.
- Manage a highly complex and specialist clinical caseload independently, dealing with patients’ needs in a community setting. Where appropriate refer on for care or delegate to other members of the clinical or administrative team.
- Using shared decision making to assess, plan, implement and follow-up biopsychosocial assessments and treatment plans to enable patients to live fulfilling and productive lives according to their goals and abilities.
- Working with the Community Nurse Practitioner and Care Coordinators, take responsibility for ensuring all the care plans of the identified cohort of patients are updated and their Long Term Condition care is up to date.
- Attend the practice MDT monthly to discuss patients from the caseload and be part of the AgeWell MDT on a weekly basis.
- Work in partnership with multi-disciplinary team colleagues, physiotherapists and social workers, alongside the patients' families, teachers, carers, and employers in treatment planning to aid rehabilitation and to reduce fragmentation of care
- Working with the Care Coordinators, perform discharge reviews and develop discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, and community settings.
- Advise patients, and their families or carers, on specialist equipment and organisations that can help with activities of daily living and support the learning of the Care Coordinators to be able to advise patients similarly.
- Help patients to adapt to and manage their physical and mental health long-term conditions, through the teaching of coping strategies
- Take the lead in standard setting for occupational therapy practice in general practice as designated
- Maintain an up to date knowledge of self-management advice, specialist equipment, environmental adaptation, technology and legislative responsibilities which may support increased function and/or management (for onward referral / request to secondary care)
- Ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve practice and PCN and objectives.
Leadership, Training and Mentoring Role:
- To act as a clinical leader in the delivery of care Coordination and Frailty services across practice sites
- Actively maintain, monitor and audit the standard of care provided, with reference to clinical governance, risk management, documentation and client records
- Implement agreed research based recommendations to improve care
- Be involved in the teaching of staff members, students and members of the multi- disciplinary team and others as appropriate
- Set up, maintain and organise agreed training and development programmes in line with the Practice policies
- To actively take part in the recruitment and selection to all posts that fall within the remit of the Care Coordination team
- To assist the management when needed, in recommendations to the Practice Manager on remunerations
- To lead on the annual appraisals of the Care Coordination team, collating training needs highlighted during the appraisal process
- To work with management on any identified performance issues of individuals within the Care Coordination team
- Ensure that there is effective communication between the Care Coordination team and other teams of the practice using the systems in place, to allow staff to keep abreast of patient care and practice or PCN developments
- To liaise with line managers on operational issues that involve the Care Coordination team

The postholder will have access to appropriate clinical supervision within the practice to provide general advice and support on a day to day basis.

Essential
BSc in Occupational Therapy under a training programme approved by the college of OTs
Health & Care Professions Council (HCPC) Registration
Able to operate at an advanced level of clinical practice
Evidence of appropriate continuing professional development and education to maintain up to date knowledge

Pre-reg MSc in Occupational Therapy under a training programme
