Job description

To be an involved and contributing member of the multi disciplinary team (which includes RMNs, social worker, occupational therapist, psychiatrist, psychologist, psychosocial workers and team administrators). Responsible for the assessment, review, risk assessment and case management of patients with varying levels of complexities. Communicate and work collaboratively with other professionals in providing holistic care to meet patient needs. Identify and provide evidence based interventions appropriate to patient needs.
