To provide responsive and expert clinical care for adult patients within the Community Hospital Inpatient setting.
The purpose of the post is to lead in the development of care for older people with multiple chronic diseases who are hospital in-patients, through proactive management support their effective discharge to a suitable place.
This will involve working collaboratively with the multidisciplinary team to establishing a whole systems approach to managing care for these individuals.

The nurse is based within a Locality Hospital site and will have overall responsibility for patients based on the wards who require support and management to facilitate a safe discharge home.
