Job description

Management of patient health and illness Accurately examine and assess patients with an undifferentiated diagnosis, from a physiological, psychological and social perspective, and plan clinical care accordingly. Manage and support patients with chronic long term conditions working with the wider services and healthcare professionals to access other care pathways as required. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Support patients in care and nursing homes and visit patients in their homes as required.

Provide information and advice on prescribed or non-prescription medication, on medication regimens, side-effects and interactions. Ensure full and accurate documentation in patient notes of clinical history, physical examination, diagnosis and treatment plan. Where the post holder is an independent prescriber, to prescribe safe, effective and appropriate medication as defined by current legislative framework. Professional role To promote and take responsibility for own role, personal development and clinical excellence.

Working with the clinical team and others in developing roles and responding to changing healthcare needs. To facilitate and participate in multi-disciplinary education and clinical supervision where necessary. To work to NSF, NICE Guidelines, evidence-based care and the scope of any relevant professional practice / bodies. Participate in organisational decision making, interpret variations in outcomes and use data from information systems to improve practice.

To maintain own professional development to work autonomously within professional boundaries and take responsibility for own learning and performance. To promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of one own clinical practice through the quality assurance strategies of audit and peer review. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.

Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation. Pro-actively promote the role of the ANP within the PCN, and externally to key stakeholders and agencies. To play an active role in the development of PCN development plans and new services. Ensure awareness of sources of support and guidance (e.g.

PALS) and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate. Team role To liaise and work with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector and CCG in order to assure appropriate care is provided for the PCN population. To participate as a key member of the multi-professional team through the development of collaborative and innovative practice. To value all team members and encourage an environment that supports peer review / peer learning.

To encourage self-assessment and challenge any complacency or actions that are not in the interest of the public and/or users of services. To anticipate barriers to communication and take action to improve communication. To support the delivery of health and safety guidance as agreed within the PCN. To support the writing of maintenance and delivery of CQC guidance within the PCN.

Organisational To complete records, audits, reports and respond to appropriate questions and requests. To attend meetings as requested. To keep the Primary Care Network Manager informed about pressures and difficulties or problem areas when aware. To contribute to the PCN strategy and developments when requested, including the contribution to the development of PCN protocols, policies and standards.

To work according to Clinical Governance and support the Clinical Governance Agenda. To contribute to risk assessments, audits and Significant Events and keep informed of the importance of Infection Control.
