The Integrated Discharge Service are seeking an enthusiastic and pro-active discharge co-ordinator to join the team.

The post holder will will provide crucial support within the discharge service and across Manchester Royal Infirmary to facilitate safe, person centred and timely patient discharges.

You will be willing to adapt to change and learn new skills within a specialist service that positively contributes to better patient outcomes and experience.

  • Identification of medically optimised patients.
  • Reporting any delayed discharges in line with Reason to Reside criteria.
  • Effective communication with various departments and disciplines to ensure safe and timely discharges can be achieved.
  • To be competent with IT systems.
  • Establish partnerships and networks with wider service providers and agencies.
  • To work collaboratively with all members of the hospital discharge service.
  • Effectively communicate with patients, families, other services and wider networks.
  • Assisting patients and carers to access information, or signpost as required.
  • To have an understanding of the Social Care requirements and legislation, including the Mental Capacity Act and Care Act.
  • To provide a safe and proactive contribution to patient flow in the organisation.
