We have an excellent opportunity for a ASYE Social Worker to join the Lower Valley Personalised Long Term Support Pod based at the Elland Library. There is also access to other touch down points within the local area including the Shay stadium. The Lower Valley Pod covers areas around Elland, Brighouse and Hipperholme.
Calderdale Adult Services have a dedicated Personalised long term support team supporting adults age 18 years +. We believe ensuring the health and well-being of people is more important than ever as adults with increasingly complex needs and frailty are often more vulnerable members of society. What Adult Services seeks to achieve by recognising the interaction between physical, mental, and social care factors is to ensure independence of adults and their quality of life.
Calderdale’s approach to Social Work is to be creative, proactive, preventative and personalised with a focus on holistic strengths-based assessment. The council’s overriding ethos is to engage with adults, carers, and their families through a shared conversation model. This is a new way of working which encourages professional engagement in the ‘persons’ story and a move away from the process led assessment tool.
Our social work practice in Calderdale is:
  • Human rights based with a key emphasis on the Mental Capacity Act 2005 and subsequent Deprivation of Liberty Safeguards.
  • Underpinned by values of social justice, the idea of fairness between the individual and society.
  • Ensures individual rights to access health funded services is assessed under the NHS Continuing Health Care Framework (amended in 2018).
  • Person-centred when approaching the safeguarding of a vulnerable adult.
  • Positive about enabling ‘risk’ considering prevention and the management of risk.
You will be supported in your professional role to:
  • Develop multi-agency links while increasing confidence in leading multi-disciplinary meetings i.e., health, social, housing, police, therapy
  • Undertake safeguarding duties under s42 of The Care Act 2014
  • Planning discharges for individuals who are an inpatient in local and out of area hospitals.
  • Complete statutory assessments, understanding eligibility and support planning which is outcome focused.
  • Work with carers, considering eligible needs and support planning
  • Supported to complete ASYE, linking with the Authority wide ASYE colleagues, your ASYE colleagues with our service area and one to one mentoring from an experienced social worker.
The role requires you:
  • Take accountability for self-development
  • Engage in critical reflective practice, both in 1-1 supervision and group reflect sessions.
  • Take ownership of continued professional development needs mapped against the BASWA Professional Capabilities Framework (PCF).
