Job description
To facilitate and provide specialist care to people with MS, and other neurological conditions which aims to improve quality of life, in areas across Airedale, Wharfedale, Craven and Bentham.
- Provide evidence based individualised care, for people with MS and other neurological conditions and their carers’, which empowers patients to take an active role in their care.
- To work with others to reduce the need for unplanned admissions to hospital and/or the need for a prolonged stay in hospital.
- To contribute to the development and delivery of national and local priorities for MS and neurology, working collaboratively with other neurological specialists through crossed boundary working.
- To act as a resource for all members of the MDT and provide specialist advice concerning aspects of MS and neurology management.
- To contribute to developing and delivering clinical leadership, education and training to MDT’s and all service users.
Patient Specific Objectives:
- To assess, plan, provide and evaluate nursing care to support people with MS and other neurological conditions, who often present with complex symptoms.
- Assessment and review of patients on caseload, recognising priorities and acting accordingly.
- To work in partnership with patients (and carers/families) to empower them to make informed choices about their health and their healthcare, embodying self-care ethos.
- Work closely with consultant colleagues within ANHSFT and senior nurses in the neurology specialist nursing team.
- To review the prescribed medication programme and to ensure that patients receive appropriate pharmacotherapy in effective doses, educate & support to actively engage in their agreed treatment programme.
- Be able to assess, diagnose symptoms/changes and treat/refer within agreed medical/nursing protocols & policies.
- To make decisions regarding clinical condition, care, and treatment alterations daily with the support of neurology nurse colleagues if required.
- To monitor patient's clinical status following medication changes.
- To ensure appropriate and effective communication between the patients, primary health care team, hospital, palliative care services, carers (formal and informal) and other outside agencies.

- To advise on lifestyle changes that would be advantageous to health.

- Provide palliative & supportive care to patients and their carers/families, including sensitive information regarding prognosis using advanced communication skills and support from neurology nurse colleagues (if required).
- To contribute and develop the research agenda, incorporating evidence-based practice into the management of skilled holistic care.
- To monitor, evaluate and audit the service at regular intervals, as part of an agreed programme to ensure both a high standard of care and the effectiveness of the whole service in improving health outcomes.
- To provide expert knowledge and leadership in the specialist area.
- Manage programmes of specialist care effectively through working in partnership with
- Delegating/referring aspects of care to MDT to optimise health outcomes and resource use, thus providing appropriate support to patients and carer
- Support and enable individuals, carers, and communities to take action to meet their priorities for MS care.

- Work across primary and secondary care, as well as with outside agencies to ensure continual development and improvement of service for people with MS and other neurological conditions.
