Mental Health Support Worker – Forensic Division

We provide specialist rehab, recovery and accommodation services in the southwest. We deliver community based solutions to people with complex unmet needs that are transitioning out of psychiatric hospital, or are at risk of readmission
The role
You will provide ongoing support and guidance to Male Service Users who are currently under The Forensic Mental Health Division based in Supported Community Housing in East Bristol. We ensure clients can live safely, considerately and with dignity, whilst also adhering to the terms and conditions of their licence. The properties are based in East Bristol and this role would suit somebody with access to a vehicle.
You will be working as part of an innovative and compassionate team responsible for enabling people with various and complex support needs progress towards recovery, wellbeing and independence, through utilisation of creative, flexible and outcomes-driven support planning and delivery techniques.
You will support clients with daily tasks within their supported accommodation in East Bristol, accessing the local community for appointments, shopping, and social activities. You will be required to update daily logs, risk assessments and support plans as part of your role.
Main responsibilities:
· Deliver creative, flexible and, outcomes driven support
· Undertake pre placement support tasks as directed by senior staff to facilitate smooth transition into the service
· Support spiritual, cultural and religious choices
· Enable service users to access and participate in their local community, including access to education and employment opportunities
· Provide support to maximise service user income to include benefit claims and renewals
· Facilitate weekly house meals in shared environments
· Facilitate medication regimes and compliance monitoring
· Maintain client budgeting systems and associated documentation
Hours
3 full time positions available with various shift patterns - Monday to Friday, Tuesday to Saturday, Sunday to Thursday, Weds to Saturday
