An exciting opportunity has arisen within the North Manchester Home Based Treatment team for an experienced and enthusiastic Mental Health professional to join our multidisciplinary team as a Band 6 Mental Health Practitioner - RMN / SW / OT
The Home Base Treatment Team provides a community based mental health service to people in North Manchester. We are committed to the delivery of high quality and intensive mental health care and treatment to people with severe and acute mental health needs.
Candidates will be expected to work as as part of an MDT. You will have strong skills, knowledge, and experience of working with patients with mental health needs. Your skill set will include mental health assessments, risk assessment and management plans, and be able to formulate collaborative care plans with service users.
The successful applicant will also be actively contributing to ongoing service developments.
Hours of work will include day duties and nights on rotation, working full time.

The post holder will have experience of clinical practice within the urgent care setting, preferably with prior experience of community based urgent care services / community mental health services. The post holder will agree a plan of care collaboratively with service users and their carers. The service user will follow agreed practice guidelines at all times and work as part of a multi-disciplinary team. The post holder should be committed to delivering high quality of mental health care and contribute to ongoing service development. The post holder will be expected to cover all duties across a 24/7 service on a rotational basis.
