Norfolk and Suffolk NHS Foundation Trust provides child and adult mental health services, learning disability, wellbeing, older people's and eating disorder services across Norfolk and Suffolk.

Job overview

There is an opportunity for a Band 5 Mental Health Nurse to join the In-reach/Outreach service at Carlton Court Hospital. We are seeking a motivated and enthusiastic individual who have a passion for supporting people with dementia who are living in care homes. The role will also involve supporting and educating staff in the homes, case management and leading formulation meetings. You will work alongside registered and unregistered staff, plus a clinical psychologist.

The focus of the team’s approach will mean using a psychological model which will encompass utilising skills of observation, communication and the ability to collaborate with others. We aim to develop strategies in order to support people with dementia who are experiencing distressed behaviours and behaviours that the care home staff find challenging.

Main duties of the job

  • Supporting senior staff with case management
  • Observation and information gathering about the service user
  • Utilising ‘The Newcastle Model’ Framework
  • Coaching and education of staff within care homes
  • Care planning and developing behaviour support plans
  • Building rapports and positive working relationships with care home staff
  • Attending team meetings and participating in reflective discussions
  • Gathering life story and utilising this within a plan of care

Experience of working with the person with dementia is essential. There will however be the opportunity for self-development within a very supportive and positive team. We cover the Lowestoft, Waveney and Great Yarmouth area.
