The Forensic Outreach Team in Leeds is based at the Newsam Centre and supports service users throughout the city. Our service users have previously committed offences as a result of their mental health difficulties, and have usually had very lengthy hospital admissions prior to being discharged to the community. We work closely with medical, psychology and social work colleagues, along with our statutory and third sector partners, to safely support service users make this transition from secure care settings to community living.
We are looking for dynamic and motivated individuals to take on the role of Band 6 Community Mental Health Nurse. The ideal candidate will be thorough, enthusiastic, emotionally resilient, and as part of the forensic multidisciplinary team be able to work with individuals who challenge services. Previous experience of community work would be an advantage but is not essential as you will receive a thorough induction and training to support your development.
The successful candidate must have current NMC registration, a full UK driving licence, and access to a vehicle for work use.
You will be providing care co-ordination and specialist nursing interventions to a small community caseload across Leeds, supported by the MDT. You will carry out a variety of assessments to inform complex care planning, with the goal of enabling our service users to safely manage their mental health conditions. You will provide reports and evidence for legal proceedings, liaise with colleagues in other services, and work holistically with service users and their carers to build engagement and work towards discharge from forensic services. You will also work with service users as they prepare to leave hospital, to ensure seamless transition of care to the community.

You will be responsible for managing your own caseload, prioritisation of workload, and time management, working autonomously and flexibly to meet the needs of the service. You will provide specialist nursing assessment and therapeutic interventions to service-users with a history of offending linked to their mental disorder, supporting them to safely manage their risk and maximise their recovery in community settings.
You will prepare and present reports for court and tribunals, and communicate complex information sensitively to carers, services users and other agencies.
You will actively contribute to MDT decisions regarding formulation, risk assessment, management, and positive risk-taking, and be able to deliver creative interventions/packages of care to meet the complex needs of individual service users leaving low and medium secure care, in diverse settings.
You will independently make decisions in relation to action to be taken as a result of service user contact/non-contact. This is a continuous part of day to day activity e.g. face-to-face contact, managing concerns reported by other disciplines, agencies, carers etc., and will involve a range of complex situations.
